About this worksheet
A help-wanted note, a card about a made-up person and a job application form. Students copy the right detail onto each line, which builds the habit of matching labels to information before they fill in a form about themselves.

What each level includes
- Level 1: A short form: job, name, address and phone.
- Level 2: Add email, days available and last job.
- Level 3: Add dates, a reference and a short answer about why you want the job.
Ideas for using it
